1. For First Time Applicants:
- Passport application form duly accomplished, typewritten or
printed.
- Original/Certified True Copy and Photocopy of the Birth Certificate
issued by the Local Civil Registrar or the National Statistics
Office.
- For an applicant who is in the U.S. Military Service, a certification
from the Commanding Officer concerning his/her enlistment and
status.
- Plus the additional requirements
listed below.*
2. For Renewal of Expired or Expiring Passport
- Passport application form duly accomplished, typewritten
or printed.
- Old Passport with photocopy of the data and photograph page(s).
If not applying in person, include pages showing latest arrival
stamp by US Immigration.

*ADDITIONAL REQUIREMENTS TO BE SUBMITTED BY ALL APPLICANTS:
a) Two passport size photographs, 2 x 2 inches, colored or black
and white with plain white background, taken full front view
with the applicant dressed in decent attire. If taken with eyeglasses,
photographs must bear no reflections. Photographs of male applicants
wearing earrings are not acceptable.
b) Back to back copy of Resident Alient Card (Green Card) or
Temporary Resident Card of Employment Authorization card or copy
of appropriate visa. Originals must be presented for verification.
c) For applicants who were married outside of the Philippines,
a Report of Marriage must be filled up with the following additional
requirements:
- Marriage Certificate
- Two (2) Photos of husband & wife
- Three (3) copies of IDs of both husband & wife (e.g. Driver's
License, Passports)
- In case of female applicants, birth certificate
3. For replacement of Lost Passport
- Passport application form duly accomplished, typewritten or printed.
- Original/Certified True Copy and Photocopy of the Birth Certificate
issued by the Local Civil Registrar or the National Statistics
Office.
- Affidavit of Loss stating the true circumstances surrounding
the loss which must be duly notarized by a Notary Public.
- Applicant must apply in person for interview.
NOTE: Pursuant to existing regulations, the application must
undergo a 15-day waiting period from the date of submission before
a replacement passport can be be issued.
OTHER REQUIREMENTS:
For female applicants who were married and later divorced by their
foreign spouses:
- Copy of the Divorce Decree
- Birth Certificate
- Marriage Certificate
For widows who have remarried:
- Death Certificate of husband
For adopted applicants:
- Original/Certified True copies and photocopies of the Birth Certificates
duly registered (with Registration No./LCR No.) amended and signed
by the Local Civil Registrar BEFORE and AFTER adoption.
- Original/Certified True copy and photocopy of the Court Order
on the adoption.
PASSPORT FEES:
- Renewal or First Time Passport $ 60.00
- Amendment of Passport $ 20.00
- Replacement of Lost Passport $100.00 which
includes a $10.00 service charge
(For applications sent in by mail, enclose a self-addressed
stamped envelope with $3.20 postage along with the correponding
payment in the form of cash, money order, bank draft or cashier's
check.)
Date of Release : Documents may be picked up from the Front Desk
between 3:00 to 4:00 pm the following working day and upon presentation
of the claim stub.
FOR REGISTRATION OF CHILDREN BORN ABROAD OF FILIPINO PARENT OR
PARENTS:
Children born of Philippine parent or parents must be registered
at the nearerst Embassy or Consulate of the Philippines with one
(1) month after birth. To register, submit the following:
- Three (3) copies, DFA Form No. 40.
- Left and right footprints of the child must be imprinted at
the back of the form at the Consulate.
- Two (2) passport size photographs of the child.
- Four (4) copies, birth certificate (original copy must be
presented for verification)
- Four (4) copies, pages 1 and 2 of passport of Filipino parent
(original must be presented for verification)
- Marriage Certifiate of Parents.
- If illegitimate, Certificate of Illigitimacy from mother,
duly notarized.
MANDATORY REGISTRATION OF FILIPINO NATIONALS OVERSEAS
Pursuant to provisions of Republic Act 8042, otherwise known as
the Migrant Workers and Overseas Filipinos Act of 1995, all Filipino
Nationals are required to register with the Philippine Consulate
General. Certificates of Identity and Registration will be issued
FREE OF CHARGE.

TAX REMINDER:
All Filipinos gainfully employed at home or abroad are required
by law to file their Philippine Income Tax Returns every year.
When filing their Income Tax, attach W-2 form and 1040 for every
tax year. Income Tax payment is payable to the Philippine Consulate
General in Cash, Money Order, Bank Draft or Cashier's Check.
